Please use the document below to apply for a Public Defender. You can return the completed form to the Office of the Public Defender during regular business hours, scan and e-mail the completed application to: PublicDefender@lebanoncountypa.gov, or fax the completed application to 717-228-4464. Please make sure the application is complete. AN INCOMPLETE APPLICATION WILL NOT BE REVIEWED.
Your application will not be reviewed until we have proof of income. The documentation needed to demonstrate your income is listed under the Eligibility tab on our main page. If you are currently not employed, you will need to fill out an Affidavit of Support to prove to the court that you are unemployed and unable to pay for a private attorney. Please print out the form below and bring it to a notary. Do not fill out the form nor sign the form until you are in front of a notary.
