We use the "Five Wishes" documentation to assist you in these important decisions:
1. You will need to name someone as your Health Care Agent
2. You will need two Witnesses to sign the document
3. The document does not need to be notarized.
4. The "Five Wishes" documentation is recognized throughout the state of Pennsylvania.

- Learn where to start these conversations with your children, your parents, or the most important people in your life, and your doctors. Understand what the documents are that you need and how to use them. This is a conversation often avoided in our community until it is too late to help.
- Work with a trained facilitator to create your own documents or review your documents you have previously created. Understand how to pick the right people to speak for you when you cannot speak for yourself due to a health crisis and outline their role and responsibilities for them.
- Review and understand why updating your documents with loved ones and your health care professionals is vital as life situations change. You will receive a free copy of Five Wishes, a booklet containing both a Living Will and Power of Attorney for Healthcare.
